A baby is in fluid within the amniotic sac and its lungs are full of that fluid. When a baby is born it is squeezed through the birth canal amd that helps expel the majority of that fluid. It is then triggered to breathe by stimulation. Stimulation can be from exposure to air, temperature change, or from being handled.
When a baby is born into water it is to them a calm, peaceful, non traumatic experience. Just a simple transfer of space and fluid a.d then is gently brought to the surface and takes its first breaths and their lungs expand. Until then, however, the umbilical cord is still receiving oxygenated blood profused by the placenta for several minutes. In order for a baby to drown during a water birth it would have to be held under water until it was no longer receiving oxygen, or a cord was snapped or clamped and the baby held under still.
Waterbirth is safe and a wonderful thing for both mother and baby. The reason most people shy from it is the question you asked and that it can be difficult to ascertain how much blood loss there is in water if you aren’t as familiar with waterbirth. So just find someone who routinely does waterbirth
